Where the cramps-and-dehydration myth came from

In the late 1990s and early 2000s, as creatine exploded in popularity among team-sport athletes, anecdotal reports began circulating that it caused muscle cramps, dehydration, and heat problems. Some sporting bodies issued cautious warnings, and a few even discouraged creatine use in hot conditions. The concern had a plausible-sounding mechanism behind it: creatine draws water into muscle cells, so the worry was that this intracellular water came at the expense of fluid elsewhere in the body, thinning out the blood and other compartments and impairing the body's ability to cool itself and stay hydrated.

It was a reasonable hypothesis. It just didn't survive contact with the evidence.

The most thorough look at the heat-and-hydration question is a 2009 systematic review with meta-analysis published in the Journal of Athletic Training. Pooling the controlled studies that measured hydration, thermoregulation, and heat tolerance in creatine users - in both temperate and hot environments - its conclusions were unequivocal: creatine did not reduce total body water as a proportion of body mass, and it did not impair the body's thermoregulatory responses during exercise in the heat.[1]

Perhaps the most telling test came from a study on the exact worst-case scenario the myth worries about: dehydrated men performing prolonged exercise in the heat. Even there, a week of creatine did not compromise hydration status, did not impair thermoregulation, and did not increase the incidence of heat-illness symptoms - and cramping was not increased.[2] A dedicated 2008 review put it bluntly in its very title, describing the cramps-and-dehydration idea as a myth to be put to rest.[2]

The International Society of Sports Nutrition's position stand reaches the same conclusion, stating that creatine does not increase the incidence of cramping, dehydration, or muscle injury - and may in fact help reduce them.[3]

Why the myth is backwards

Here's the part that flips the whole worry on its head. Creatine is an osmolyte - it attracts water - so supplementing increases the water held inside your muscles and raises your total body water. The original fear assumed this water was being stolen from your bloodstream and other compartments, dehydrating them. But the research shows total body water goes up, not down, and the balance between fluid inside and outside your cells is maintained. In other words, rather than drying you out, creatine tops up your body's water reserves. Some researchers have even suggested this extra intracellular water may help with thermoregulation during exercise in the heat, acting as a bigger buffer against fluid loss - the opposite of a dehydration risk.

So what actually causes cramps and dehydration?

Exercise cramps and dehydration are real - they're just not caused by creatine. The genuine culprits are the familiar ones:

  • Not drinking enough fluid to replace what you lose through sweat, especially in hot or humid conditions.
  • Heavy sweat and electrolyte loss, particularly sodium, during long or intense sessions.
  • Muscle fatigue and overexertion - working a muscle beyond what it's conditioned for is a leading trigger for exercise-associated cramps.
  • Heat itself, which stresses the body's cooling and fluid-balance systems.

Address those - sensible fluid intake, electrolytes on long hot sessions, and training within your capacity - and you're managing the actual causes, rather than blaming a supplement that isn't responsible.

Should you drink more water on creatine?

There's no need to force down extra litres because of creatine specifically, but staying well hydrated is simply good practice for anyone who trains - creatine or not. The modest amount of water creatine holds is stored in your muscles, so keep drinking to thirst, drink a bit more around hot or long sessions as you normally would, and you'll be fine. Creatine doesn't create a special hydration demand; it just quietly tops up the water your muscles hold.
